Covid-19 Vaccination Clinic at FHS
Attendees: Open for all FPS students ages 16 and older and FPS staff members
Dates:
- First Dose given on April 27 & 28 8:30am-11:00am
- Second Dose given on May 18 & 19 8:30am-11:00am
- (Graduating seniors may return to FHS)
Vaccine: Pfizer 2-dose series
Provider: Medical Arts Pharmacy
Location: FHS Gymnasium
REQUIRED FORMS:
Communication: Email will be sent to parents with attached consent form and FERPA form and they must be returned by Friday, April 23 to the school nurse via email or in person at the nurse’s office. We will send reminders to all attendees who receive vaccination of their second vaccination date.
